What we collect
- Contact details you choose to provide: name or preferred name, phone number, email address
- The borough or area you name and the general type of help you are seeking
- Your preferred contact time and any optional message you write
- Basic technical information created when any website is visited
What we ask you not to send
Please do not include diagnosis details, substance use history, insurance member IDs, date of birth, or other sensitive medical or insurance information in our web form. Our general web forms are not designed for sensitive health information.
Why we collect it
To respond to your request, to explain relevant options and public resources, and — only with your explicit permission — to help you connect with a provider you have chosen.
Consent before anything is shared
We do not share your information with a treatment provider unless you have agreed to it first, after we have told you what would be shared and with whom. You can decline, limit, or withdraw that permission at any time, and you can always contact providers directly yourself.
Calls
If a call is ever recorded or monitored, we will tell you at the start of the call. You can ask about the recording status of any call before you continue.
Health privacy laws
We are an information and navigation resource, not a healthcare provider, and we do not claim HIPAA or 42 CFR Part 2 status. We still treat what you share with us carefully and limit it to the purpose you gave it for.
Retention and your choices
We use your information only to respond to your request, and we do not sell personal information. You can ask us at any time to correct or delete the information you have sent us.
